These Terms and Conditions ("Terms") govern your use of the services, clinic, mobile appointments, and website of Revive IV Hydration & Wellness, operated by Revive IV Hydration LLC ("Revive," "we," "us," or "our"). By booking an appointment, receiving a service, or using our website, you agree to be bound by these Terms.

1. Services Provided

Revive offers IV hydration therapy, vitamin injections, medical weight loss programs, hormone replacement therapy, wellness services (including infrared sauna and NAD+ therapy), hair and beauty services, and diagnostic testing services. All clinical services are administered by licensed Registered Nurses (RNs), Nurse Practitioners (NPs), or Physician Assistants (PAs), under the supervision of a board-certified Medical Director.

2. Medical Screening and Eligibility

A Good Faith Exam and health history review with a licensed Nurse Practitioner is required before your first treatment and periodically thereafter as clinically appropriate. Revive reserves the right to decline, delay, or modify any service based on your health history, current condition, lab results, or clinical judgment of our providers.

3. Appointments and Mobile Service

Appointments may be scheduled at our Roanoke clinic or as a mobile visit within our designated service area (generally a 30-minute radius of Roanoke). Mobile appointments may be subject to a travel fee. You are responsible for providing a safe, accessible location for mobile services and for accurately disclosing relevant medical information at the time of booking.

4. Cancellations and Refunds

Cancellations, rescheduling, no-shows, and refunds are governed by our separate Cancellation & Refund Policy, which is incorporated into these Terms by reference and available at [insert URL] or upon request.

5. Payment

Payment is due at the time services are rendered unless other arrangements have been made in writing. We accept major credit/debit cards and HSA/FSA cards for eligible services. Prices for services, packages, and memberships are subject to change without notice; current pricing is provided at the time of booking or upon request.

6. Assumption of Risk and Informed Consent

All medical and wellness services carry some degree of risk. Prior to treatment, you will be asked to review and sign a service-specific informed consent form describing the nature of the treatment, expected benefits, potential risks and side effects, and alternatives. By proceeding with a service, you acknowledge that you have had the opportunity to ask questions and that you consent to treatment.

7. Medical Disclaimer

Use of our services is subject to our Medical Disclaimer, incorporated into these Terms by reference, which describes the limitations of our services and clarifies that they do not replace ongoing care from your primary care physician.

8. Age Requirement

You must be at least 18 years old to receive services independently. Minors may only be treated with the written consent of a parent or legal guardian, where permitted by Virginia law and at the discretion of our clinical staff.

9. Privacy of Health Information

Our collection, use, and disclosure of your protected health information is described in our HIPAA Notice of Privacy Practices, incorporated into these Terms by reference and available on our website.

10. Limitation of Liability

To the fullest extent permitted by law, Revive IV Hydration LLC, its owners, employees, and contracted providers shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our services, except to the extent caused by our gross negligence or willful misconduct. Nothing in these Terms limits liability that cannot be limited under applicable law, including claims for professional negligence governed by Virginia medical malpractice law.

11. Website Use and Intellectual Property

All content on our website, including text, graphics, logos, and images, is owned by Revive IV Hydration LLC or its licensors and may not be copied, reproduced, or distributed without prior written permission.

12. Governing Law

These Terms are governed by the laws of the Commonwealth of Virginia, without regard to its conflict-of-laws principles. Any disputes shall be resolved in the state or federal courts located in Roanoke, Virginia.

13. Changes to These Terms

We may update these Terms from time to time. The "Effective Date" above reflects the most recent revision. Continued use of our services after changes are posted constitutes acceptance of the updated Terms.
